Wellness Calls

Daily Check-Ins with Your Loved Ones

The need to connect with someone is what makes us human. To help provide needed reminders or companionship, we have developed the Caring Hearts® Wellness Calls as part of our comprehensive in-home care services. Many of our clients don’t require regular visits from our team yet but could benefit from helpful check-ins to ensure they are doing okay and aren’t in need of anything. These calls also allow our team to connect with them and give your loved ones an opportunity to socialize.

  • We offer wellness calls for a variety of reasons, including:
  • Meal reminders
  • Medication reminders
  • Morning or evening check-ins
  • Hydration reminders
  • Exercise reminders
  • Companionship and connection

These calls can happen as frequently as you would like, and we can arrange them to fit you or your loved one’s schedule.

Responding to Unanswered Calls

In some instances, our clients don’t always answer our calls. In most cases, there is no reason to worry. However, we prefer to have a backup plan for these situations. Our Caregivers will work with you and your loved one to create a customized response plan for when there is no answer. Many response plans involved calling or emailing a family member so that they can try and get in contact with their loved one.

Wellness calls can help minimize the chances of hospital admittance. Combined with our wearable technology that alerts us if your loved one falls, you can rest assured that they are fully taken care of even when you can’t be with them.

Caring Hearts offers FREE In-Home Care Assessments where we will discuss our 24-Hour Monitoring Services as well as our other in-home care services. We use this no-obligation meeting as an opportunity to learn about your needs and allow you to learn about us.

Steps to Care

  1. 1.
    Initial Call Once we’ve received your information, we will have a short 5-10 minute call to answer any pressing concerns and set up a time to meet in person.
  2. 2.
    In-home Care Assessment Here we will answer all your questions, and ask some of our own to craft a Care Plan customized to your unique needs.
  3. 3.
    Start of Care We will come with our Caregiver on the first day of care to go through the Care Plan together and help with introductions.
  4. 4.
    Client Visits Once we have begun care, we will pop in at times our Caregiver is there to see how things are going and determine if we need to adjust the Care Plan.
