Veterans Care Coordination

Through our partnership with Veterans Care Coordination, we’re able to assist qualifying veterans and their spouses to apply for and obtain the Veterans Aid and Attendance Benefit to help cover their medical expenses, including Caring Hearts in-home care services.

National Certification Board for Alzheimer Care

The National Certification Board for Alzheimer Care (NCBAC) is the ONLY independent national board that follows the healthcare certification/licensure model to administer standardized certification examinations in dementia care. The purpose of the Board is to provide a benchmark for core competencies and knowledge for those who provide care and education in the field of dementia.

In order to serve a growing senior population, AoA envisions ensuring the continuation of a vibrant aging services network at the State, territory, local and tribal levels through funding of lower-cost, non-medical services and supports that provide the means by which many more seniors can maintain their independence.

Aging USA

Aging USA provides action-oriented educational resources offering practical and spiritual guidance on caring for aging or ill family members and friends. Its elder care resources and programs are designed to help caregivers develop the knowledge and skills they need to embrace caregiving and make a difference in the lives of the aging and chronically ill.

Alliance for Aging Research

The private, not-for-profit Alliance for Aging Research is a national citizen advocacy organization working to improve the lives of Americans as they grow older by advancing biomedical and behavioral research in aging and health.

Alzheimer’s Association

The Alzheimer’s Association is the leading global voluntary health organization in Alzheimer care and support, and the largest private, nonprofit funder of Alzheimer’s research. Their annual Walk to End Alzheimer’s is the world’s largest event to fight Alzheimer’s disease, with over 600 Walks nationwide. Caring Hearts is proud to organize teams across the country for Walks each year.

American Association for Geriatric Psychiatry

The American Association for Geriatric Psychiatry is a national association representing and serving its members and the field of geriatric psychiatry. It is dedicated to promoting the mental health and well being of elderly people and improving the care of those with late-life mental disorders. AAGP’s mission is to enhance the knowledge base and standard of practice in geriatric psychiatry through education and research and to advocate for meeting the mental health needs of older Americans.

American Cancer Society

The ACS is a nationwide, community-based voluntary health organization dedicated to eliminating cancer as a major health problem. Headquartered in Atlanta, Georgia, the ACS has 12 chartered divisions, more than 900 local offices nationwide, and a presence in more than 5,100 communities.

The American Diabetes Association’s mission is to prevent and cure diabetes and to improve the lives of all people affected by diabetes.

The American Geriatrics Society

AGS is a not-for-profit organization of nearly 6,000 health professionals devoted to improving the health, independence and quality of life of all older people. American Society on Aging

Steps to Care

  1. 1.
    Initial Call Once we’ve received your information, we will have a short 5-10 minute call to answer any pressing concerns and set up a time to meet in person.
  2. 2.
    In-home Care Assessment Here we will answer all your questions, and ask some of our own to craft a Care Plan customized to your unique needs.
  3. 3.
    Start of Care We will come with our Caregiver on the first day of care to go through the Care Plan together and help with introductions.
  4. 4.
    Client Visits Once we have begun care, we will pop in at times our Caregiver is there to see how things are going and determine if we need to adjust the Care Plan.
